CBD is one of over 100 phytocannabinoids identified in Cannabis, which account for more than 40% of the plant’s extract. CBD does not induce intoxicating feelings, euphoria, or feelings of being high. CBD has a much lower affinity for the CB1 and CB2 receptors than Δ9-tetrahydrocannabidiol (THC), the compound responsible for the intoxicating effect found in Cannabis. However, because of CBDs wide-array of positive pharmacological properties it has been implicated to have anti-inflammatory, anxiolytic, antipsychotic, and neuroprotective effects. CBD was first discovered in 1940 and has been shown to be well-tolerated in humans with mild adverse events.

CBD has recently been studied clinically in several diseases including addiction, epilepsy, Dravet’s syndrome, Parkinson’s disease (PD), dystonia, and Huntington’s disease (HD). The mechanism of action for CBD is not fully understood due to its complex pharmacology. CBDs anti-inflammatory and antioxidant properties may be key to its wide range of potentially beneficial effects.

CBD has been shown to have several molecular targets including antagonism of the CB1 and CB2 receptors. CBD is also an agonist of the vanilloid receptor TRPV1 which is implicated in many pain mechanisms including the mechanism of action for onabotulinumtoxinA (BOTOX) in pain associated with chronic migraine. CBD has been shown to mediate neurotransmission via the serotonin receptor 5-HT1A. CBDs anti-inflammatory properties are further highlighted by its ability to activate the peroxisome proliferator-activated receptor γ (PPARγ) which is involved in glucose metabolism and lipid storage. Moreover, CBD has been reported to act directly on mitochondria by increasing the activity of mitochondrial complexes I-IV. CBD treatment decreases the level of the pro-inflammatory cytokines IL-1β, TNF-α, IFN-β, IFN-γ, IL-7, IL-6 all of which are implicated in most progressive neurological diseases such as multiple sclerosis (MS).

Pre-clinical and clinical evidence for the anti-inflammatory and anti-oxidative highlights the potential of CBD for various beneficial uses. CBD s shows great promise in numerous disease states and its role as a neuroprotective agent. New clinical studies are currently on-going in this burgeoning field of CBD science.
